ECFMG Sponsor Notes Explained

What an ECFMG Sponsor Note in the World Directory means, how a school obtains one, and why it is a first step toward US training rather than proof of quality or eligibility to practice.

Updated October 13, 2026 · WorldMedicalSchools.com editorial team

Anyone who studies medicine outside the United States and hopes to train there later will meet the phrase "ECFMG Sponsor Note." It is a short notation in a school's World Directory of Medical Schools (WDOMS) listing, and it matters a great deal for one specific purpose: whether the school's students and graduates can apply for ECFMG Certification. It is easy to read more into it than it says. ECFMG does not accredit medical schools, and a Sponsor Note describes ECFMG's requirements for graduates, not the quality of the school.

Who ECFMG is and what certification is for

ECFMG is a division of Intealth. ECFMG Certification is, in its own words, the standard for evaluating the qualifications of international medical graduates (IMGs) entering the US health care system. ECFMG lists three main uses:

  • the Accreditation Council for Graduate Medical Education (ACGME) requires IMGs entering ACGME-accredited residency or fellowship programs to be ECFMG-certified;
  • certification is one of the eligibility requirements for IMGs to take Step 3 of the United States Medical Licensing Examination (USMLE);
  • US medical licensing authorities require IMGs to be ECFMG-certified, among other requirements, for an unrestricted license.

ECFMG defines an IMG by where the medical school is located, not by citizenship. A US citizen who graduates from a school outside the United States is an IMG. ECFMG also notes a change for Canada: graduates of Canadian medical schools on or after 1 July 2025 are considered IMGs for entry into US graduate medical education.

What the Sponsor Note says

ECFMG is one of several organizations that sponsor the World Directory, and sponsors may add information on the Sponsor Notes tab of a school's listing. If ECFMG determines that a school meets its requirements, it adds a note stating that students and graduates are eligible to apply for ECFMG Certification "provided that all other eligibility requirements are met." The note also states the graduation years for which the school meets the requirements, along with other provisions.

The reverse is also stated plainly: if there is no ECFMG Sponsor Note on the Sponsor Notes tab, the school's students and graduates are not eligible for ECFMG Certification. ECFMG adds: "ECFMG does not recognize, accredit, or endorse medical schools."

Because the note is tied to graduation years, read it against your own expected graduation date, not only the school's name.

What a school must show to obtain a note

ECFMG's Medical School Listing Policy sets out the requirements. Among other things, ECFMG must have confirmation from the appropriate government authority in the school's country that:

  • the school is recognized as a medical school and permitted to operate by the government;
  • it meets governmental accreditation requirements, where they apply;
  • its degree meets the medical education eligibility requirements for licensure to practice clinical medicine in that country;
  • the program leading to the degree is not less than four credit years;
  • the school is located in that country, with its primary campuses and main operations there.

For a branch campus in another country, ECFMG may require confirmation from that country's authority too. The school must also be able to show it has the staff, procedures and facilities for its graduates to meet ECFMG's requirements, including someone authorized to verify diplomas.

How the request works

Only a school official can request a note; students and graduates cannot apply on the school's behalf. The dean signs a request form, the school supplies documentation, and ECFMG contacts the government authority to confirm eligibility. ECFMG says the time taken depends on receiving all required information, that the decision is at its sole discretion, and that a request does not guarantee a note.

Notes can be removed or modified

ECFMG may remove or modify a note for good cause based on credible information. Its policy gives examples, including false information provided to ECFMG or published on the school's website about ECFMG Certification or USMLE, no substantial medical education taking place in the host country, a host government failing to respond to ECFMG's enquiries, and no recent ECFMG activity at the school. A note you see today is not a permanent feature of a listing.

What a Sponsor Note does not do

A note is a first condition, not a pathway completed. Keep these steps apart:

StepWhat it involves
Eligibility to apply for ECFMG CertificationThe school has a current ECFMG Sponsor Note covering your graduation year
ECFMG CertificationAn application with your medical education credentials, passing USMLE Step 1 and Step 2 CK, and the clinical and communication skills requirement (met through an ECFMG Pathway, which includes the OET Medicine, or a valid pass on the former Step 2 CS)
USMLE eligibilityStudents and graduates of non-US schools must attend a school listed in WDOMS as meeting ECFMG eligibility requirements, and meet ECFMG's other criteria
Residency application and placementApplying to programs and taking part in the Match; ECFMG states an accepted, unexpired Pathway is required to participate in the NRMP Match
State licensingEach US licensing authority sets its own requirements, including ECFMG Certification among others

A Sponsor Note does not guarantee certification, a residency position, a license or employment. Each later step has its own requirements, and they change; check the current ECFMG, USMLE and state board pages directly.

The Sponsor Notes tab can also show a second, separate notation. Under Intealth's Recognized Accreditation Policy, a school that is accredited by an agency recognized by the World Federation for Medical Education (WFME) or the US National Committee on Foreign Medical Education and Accreditation (NCFMEA), and that has an ECFMG Sponsor Note, is noted as meeting the policy. Intealth states:

  • the policy does not affect anyone's eligibility for ECFMG Certification;
  • it does not affect a school's eligibility for a Sponsor Note;
  • it does not affect eligibility to take USMLE;
  • accreditation by a recognized agency does not by itself make a school's graduates eligible for ECFMG Certification; that still requires the Sponsor Note.

Intealth describes the notation as information for students, regulators and US program directors. ECFMG's policy does say that for schools whose students are not currently eligible, ECFMG may, in its reasonable discretion, require accreditation by an organization recognized by an agency that meets ECFMG's requirements. For background on agency recognition, see WFME Agency Recognition Explained.

How to check a school

  1. Search the school in the World Directory and open the Sponsor Notes tab. Remember that a WDOMS listing alone is not accreditation or endorsement.
  2. Read the graduation years in the ECFMG note and compare them with your own.
  3. Check whether the school has a branch campus or teaches part of the program elsewhere, and whether the listing reflects where you would actually study.
  4. Treat school marketing about ECFMG or USMLE with care and confirm every claim against ECFMG's own pages.
  5. If you are considering international study with a view to US training, read ECFMG's certification requirements in full before you enroll, and see our guide to investigating practice requirements.

Our methodology explains what this site records from official sources and its limits; always confirm a school's current listing in the World Directory itself.
